Generic Orthodontic Treatment Agreement - Beta
This page is for beta testing only. It is not legal advice, not a substitute for the office's approved agreement, and not a replacement for Edge.
1. Treatment and cooperation
The patient or responsible party authorizes orthodontic evaluation and treatment as planned by the provider. Treatment time can change because of growth, missed appointments, oral hygiene, broken appliances, biological response, and patient cooperation.
2. Appointments, late arrivals, and no-shows
The patient or guardian agrees to keep scheduled visits, arrive on time, and contact the office as early as possible when a visit must be changed. Missed or short-notice appointments may delay treatment and may carry a fee if included in the office's final policy.
3. Financial responsibility
The responsible party remains responsible for balances, insurance differences, payment plans, replacement items, returned payments, and services outside the agreed scope unless the final office agreement says otherwise.
4. Refunds, transfers, and early termination
Refunds or balances after early termination or transfer should be calculated under the office's written policy, considering completed services, records, appliances, lab costs, discounts, administrative costs, and outstanding balances.
5. Emergencies, repairs, and retention
The patient should contact the office for broken brackets, poking wires, aligner issues, retainer issues, trauma, or urgent discomfort. Retainers are required after active treatment, and replacement or relapse care may involve additional fees.
6. Privacy, records, and Edge cloud handoff
Codee is used here as a digital signature and PDF bridge. Edge and the office's existing cloud/workflow remain the destination. During beta, Codee may keep a private temporary bridge copy long enough to create the staff/customer handoff and 24-hour download link.
