Pre-worded templates

Orthodontic contract wording options for office approval.

These are starter templates for beta review. They are not legal advice and must be approved by Smile With Style before live patient use.

How to use

BAA first, wording second, patient use last.

The first signed client document should be the BAA/vendor approval. After that, the office can approve which contract wording should be used in the signing flow.

Simple operation: pick the template type, replace generic wording with office-approved language, verify the checklist at the bottom, then send only demo-safe contracts until the BAA, staff training, access policy, retention policy, Brevo authorization, and go-live approval are complete.
